I understand that all submissions are pending approval of the management of The TEMPLE: A Paul Mitchell Partner School. Submission of this form does not guarantee approval. Approval is pending, but not limited to, SAP report status, availability, and past advisories. All re-enrollments are limited to a start date of the first Monday of the month. Students cannot return to the program without a current, signed Enrollment Agreement.

I understand that all Re-enrollment returns will only take place on the first Monday of the month. I also understand that if any updates are needed for my Financial Aid such as updated FASFA, or required documents that this must be completed prior to my re-enrollment start date. Failure to not have this will result in the inability to return to attend the school.
